The following form is for CMU students, staff, and faculty requesting items not in the CMU Library. Institutional borrowers should use this form for their CMU Library requests.

The CMU Library is OPEN TO THE PUBLIC. ID/prox card not required. Call the Library at 204.594.0514) to be admitted.
CMU students, staff, and faculty are encouraged to have their CMU ID/prox card on hand.
